Call of Duty: Black Ops Cold War has officially arrived, and those just getting started with the campaign may have some questions about the new puzzle-based side missions. In this no-nonsense guide, we'll explain all you need to know about Operation Chaos and the evidence players must find to decrypt the Floppy Disk.

Before beginning, it's important to note that all missions in the Black Ops Cold War campaign can be replayed if you happened to miss these areas the first time. Also worth noting, it's said that collecting all the evidence, including evidence for Operation Chaos, allows you to potentially unlock a better ending once the campaign has finished. You'll also likely unlock several trophies or achievements along the way as well.

Operation Chaos Evidence locations

Coded Message: The first piece of evidence is earned in the first mission called "Nowhere Left to Run." At the end of the mission when you interrogate Quasim, make sure to select dialogue options that ensure you don't end up tossing him off the roof. For your kind behavior you'll receive the Coded Message.

Numbers Station Broadcast: About halfway through the "Brick in the Wall Mission" you'll meet a woman with an umbrella who mentions an informant stowed away in the area. This unlocks a side mission apart from the traditional quest objective that's marked by a star. Instead, drop down from the bar and follow the regular circular waypoint to a building with a blue door with number "732" written above it.

Pick the Lock, enter the room and kill the guards inside. You'll find the informant in a corner tied to a chair. Set him free and grab the audio reel on this table.

Front Page of the Observer: In order to collect this piece of evidence for Operation Chaos, you must first take pictures of at least three intel photos during the "Redlight, Greenlight" mission.

Photo 1: In the guard tower right after you take the picture of the big compound and slide down the crevice.

Photo 2: Shortly after you get to the point where you pick a lock and are discovered, Woods will save you and you'll come to a point where you see some guards and he tells you he'll approach them on your signal. Shoot them down, walk down the hallway a bit, and you'll see a door on your left. Inside is this map.

Photo 3: The third intel photo is in the top floor of the bar across from the Capital Savings building in the fake town. Go up the stairs, and you'll see this map on the wall. Provided you've taken pictures of these three photos in any order, the Front Page of the Observer will be unlocked.

Decrypting the Floppy Disk with your code

Now that you've found all three pieces of evidence to unlock Operation Chaos, your next step is to decrypt the Floppy Disk. This must be done the next time you get to the Evidence Board, so it may take a bit if you're going through the campaign naturally.

To decrypt the Floppy Disk, you must enter a broadcast frequency followed by the name of a city. To find these, scroll down to "examine evidence" to look at the three items. It may help to look at the Numbers Station Broadcast first for reference.

  • Broadcast number: Examine the front page of the Observer, and you'll see it has certain letters highlighted in red. When rearranged, these letters correspond to a city listed in the broadcast list. If your letters say New York and the listed frequency for New York is 1.0.5.8, "1058" is the first half of your password.
  • City: To get the city name, examine the Coded Message. A list of numbers will appear. You'll notice one set of numbers is red, while the other is blue. Just do some basic math to figure out the pattern in how each set of numbers ascends. In my game, for example, the red numbers increased in increments of two, while the blue numbers went up by six. The two missing numbers will correspond to the four-digit frequency of a city on your list. Enter that city name as the second half of the password.

Simply follow these steps, and Operation Chaos will be yours to play.
