If you've seen "Captain America: Brave New World" then you know the movie has a post-credits scene featuring a confrontation between the titular hero (Anthony Mackie) and the gamma-irradiated Samuel Sterns (Tim Blake Nelson). If you were confused about what Sterns was referring to, he was most likely talking about the looming multiverse threat of incursions and hinting that Captain America and others would have to fight superheroes from other universes to save the prime Marvel timeline.

In the post-credits scene, Captain America pays a visit to Sterns, presumably at the same time he visits Thaddeus Ross (Harrison Ford) in The Raft. After some back and forth, Sterns says, "It's coming. I've seen it in the probabilities, seen it plain as day. All you heroes protecting this world, do you think you're the only ones? Do you think this is the only world? We'll see what happens when you have to protect this place from the others."

If this were the MCU's Phase 1 then Sterns might be talking about aliens. But at this point in the MCU? Aliens? We've been there, done that. No, here Sterns is warning Captain America of the incursions and, more importantly, of the lengths Cap and the other heroes may have to go to protect Earth-616.

...
From "Doctor Strange in the Multiverse of Madness", we already know the MCU is experiencing incursions: events in which two universes collide, destroying one another. We've seen incursions in Marvel Comics but it isn't clear how closely the incursions of the MCU would mirror those of the source material. Sterns hints that they will soon reflect the source material in at least one way: the heroes of the MCU will have to fight, and perhaps even kill, heroes from other universes to save their own.
In the source material, the Illuminati - the only group of heroes who initially know about the incursions - are forced to confront terrible choices in order to save their world, including fighting other heroes and destroying other Earths. In one case they clash with heroes clearly meant to reflect DC's Justice League called The Great Society.
The notion that we might see the more family-friendly MCU heroes in stories in which they might have to choose between which world to destroy is surprising. We'll have to wait and see how things unfold over the next few films.
